IRS Tax Forms

The IRS requires that the PSU Foundation report payments made for rents/rentals, services, prizes and awards, payments to attorneys, medical and health care payments, and other income. The PSU Foundation is therefore required to obtain taxpayer identification numbers from all vendors and payees to whom it pays taxable income (see IRS Publication 525). The Foundation requires either a W-9 or a W-8BEN for all taxable income payments it makes.

The PSU Foundation cannot provide tax advice on the correct way to fill out tax forms. The Foundation will return all tax forms that are incomplete or completed incorrectly.

Form W-9 is required for the above-described payments for the following:

  • An individual who is a U.S. citizen or U.S. resident alien
  • A partnership, corporation, company, or association created or organized in the United States or under the laws of the United States
  • An estate (other than a foreign estate)
  • A domestic trust (as defined in Regulations section 301.7701-7)

To be valid, the W9 must include either the Social Security Number (SSN) or the Employer Identification Number (EIN) of the taxpayer listed in box 1 of the form. Your completed form will be securely delivered to the PSU Foundation. To protect your personal information, please do not email a copy of your form.

Form W-8BEN is required for the above described payments for the following:

  • An individual who is a U.S. non-resident alien. That is an individual who is not a U.S. citizen, and does not meet the IRS requirements to be a U.S. resident alien. Please see the IRS guidance on Determining Alien Tax Status for more information.
  • Invalid submissions cannot be accepted and submitters will be asked to re-complete the form. To be valid Form W-8BEN should include at least the following fields:

Part 1, Box 1: Name of the individual
Part 1, Box 2: Country of citizenship
Part 1, Box 3: Permanent residence address *This should be the payee's permanent foreign address
Part 1, Box 4: Mailing address *This should be the payee's local US address
Part 1, Box 5: US Taxpayer Identification Number *If you have a US Tax ID (Social Security Number SSN or an Individual Taxpayer Identification number ITIN), please list that number. If you do not have a US Tax ID, this field may be left blank.
Part 3: Certification: Signature of the payee

Direct Deposit/EFT Payments

The Foundation encourages all vendors to enroll in direct deposit/electronic funds transfers (EFTs) as this is the most efficient and most secure payment method. Additionally, check printing may be disrupted in the event that PSU facilities become inaccessible, while electronic payments can be processed from any location.

The Foundation has created an online direct deposit/EFT form for vendors to provide their banking information via a secure website. This form requires the upload of a voided check or other bank documentation so the Foundation can verify the vendor’s bank information.
